SlideNeighbourhood Health and WellbeingIntegrated Neighbourhood Health and Wellbeing is a place-based approach to improving health and wellbeing across Cambridgeshire and Peterborough by bringing together health, social care, local authority, voluntary and community sector (VCSE) organisations around the needs of local communities. The system is organised into neighbourhoods aligned with Primary Care Networks (PCNs), each serving a population of around 30,000 to 50,000 residents. Integrated Neighbourhood Teams work collaboratively to provide joined-up, person-centred support, combining expertise from general practice, community services, mental health, social care, public health and community organisations. By using population health data and local insight, neighbourhood teams identify the needs of their communities, focus on prevention and early intervention, tackle health inequalities, and coordinate support closer to where people live.
