ICS Outcomes Framework
Outcomes measure the whole system’s success in keeping the population well and reducing illness.
The C&P ICS Outcomes Framework is based on the core aims of our Health & Wellbeing Integrated Care Strategy, it supports our Joint Forward Plan, and it helps to guide the evolving delivery plans of our place-based partnerships.
Following extensive engagement, the ICS Outcomes Framework was agreed by the ICS Population Health Improvement Board, and subsequently by the ICB Board (May 2024).
The interactive dashboard below has been developed to show how the ICS is doing for each of the agreed measures. The dashboard will be updated on a quarterly basis. Follow the ‘click here’ links to see the latest performance data for all measures in the chosen domain. Then select a measure and click the drill down button to see more detailed information for that measure.
